Schedule an Appointment
- Call and speak with the doctor’s secretary:
- If desired, you may schedule recommended/ desired ancillary services to occur on the same day as your doctor’s visit. By verbally scheduling your appointment, important instructions will be provided to you.
- Shortly after scheduling, you will receive an email or text message with a link that is good for 24 hours, inviting you to instantly activate your “MyChart” patient portal account. To activate you will enter your date of birth and agree to the Terms and Conditions.
**Orders from non-MCH physicians may be faxed to (713) 807-4974.
Looking for a testing center near me in Houston, TX? Our Diagnostic Imaging Services department, also known as Radiology, offers a full complement of imaging modalities your non-MCH physician may have ordered – just fax those orders – with your contact information – to the number above and we’ll call you to schedule. We will also deliver results to your ordering physician.
Before Your Appointment
- Appointment Confirmation:
- You will receive a text or email confirmation the same day you schedule, and again one to two days prior to your appointment(s). The type of notification you receive will depend on how you have activated your MyChart account settings.
- If you have multiple appointments on the same day as your doctor’s appointment, you will receive separate confirmation messages from each department.
- Feeling Unwell?
- If you are sick or experiencing symptoms, please call us at 713-526-5511 and ask to speak with your physician’s nurse. If you need to reschedule, please call & ask to speak with the secretary.
- Insurance Verification:
- After scheduling your appointment, our staff will transfer you to our Business Office Registration representatives who will verify your health insurance coverage and benefits.
- If you are unable to provide your information at that time, please call our registration representatives at 713-526-5511, ext. 4624. If you have not already provided your insurance details, please do so at least 24 hours before your appointment to prevent any delays.
- As a courtesy, we will submit a claim to your health insurance company for services rendered.
- Complete Forms in Advance:
- You can save time by using e-Check-in. You will receive a text or email message with a link asking you to complete eCheck-in prior to your appointment. You can easily upload photos of your ID and insurance card(s) here. Your MyChart app offers multiple, additional capabilities.
What to Bring to Your Appointment
To help us provide the best care, please bring the following:
- Medications & Supplements: A list or photos of all medications, prescriptions, and supplements you are currently taking, including dosages. Bringing the actual prescription containers is also helpful.
- Past Medical Records: If a physician outside of Medical Clinic of Houston has recently treated you, please request they send a copy of your records to your MCH physician, or better yet, bring a copy with you.
- Medical History: A list of your current and past medical conditions, prior surgeries or treatments, and your immediate family’s medical history (parents, siblings, and grandparents).
- Identification: Your health insurance card, along with your personal and emergency contact information.
Your Appointment
We encourage you to ask any questions you have about your health, preventive or screening tests, and recommended treatments or alternatives during your visit. Our goal is to provide you with quality and compassionate healthcare, and we are here to help you become as healthy as possible.
Important Policies
- Late Arrival: If you arrive more than 15 minutes late, we will do our best to work you in, but may need to reschedule your appointment.
- Cancellations/Changes: Please call your physician’s secretary at least 24 hours in advance if you need to cancel or change your appointment.

Your physician’s staff will contact you one to two days prior to your appointment date to confirm your visit and provide clinical instructions. If you are ill or experiencing unexpected symptoms, please contact us at (713) 526-5511 and ask to speak to your physician’s nurse or clinical staff for assistance.
We strive to provide you the highest quality and compassionate healthcare. Even though these instructions provide you with information in advance of your appointment, we encourage you to ask any questions during your visit regarding your health, preventative or screening tests, and/or recommended treatments or medical alternatives.
The Clinic will verify your health insurance coverage and benefits prior to your visit. We will also file a claim with your insurance carrier once medical services have been rendered. To facilitate this service, please contact our registration representatives prior to your first visit or anytime your information has changed, at (713) 526-5511, ext. 4624 to provide us with your health insurance plan information.
